The following is information regarding practice operations, policies and procedures.

Office Hours Our office hours are Monday-Friday between 8:30 AM-5:00 PM.

Our office has five locations; Marlton, Sewell, Vineland, Burlington and Linwood/Atlantic City.

Appointments

To schedule an appointment for either location call 856-797-9161. When using voicemail, please include dates and times that work for you, as well as your name and telephone number, so we can get back to you promptly. Please remember to speak slowly and clearly.

Initial Consultation

You will need to bring with you all current insurance card(s). If this is a Workers Compensation or No Fault claim you will need to bring with your Insurance company name, address, phone number, claim number, adjuster's name and phone number.

Please bring all recent films (MRI, CAT Scans, X-rays, etc.) Note that the doctor needs to see the actual films in addition to the reports. You can save time by downloading our New Patient Forms and complete them prior to your scheduled appointment.

Prescription Refills

Our prescription policy requires 72 hours notice and can only be filled during normal business hours. (no weekends) Pain medications will not be prescribed if you have not had an office visit within the past 30 days. Refill requests can be called in at 856-797-9161. If your refill has been approved, you will be notified when it is ready to be picked up.

Medical Records

All medical records are confidential. If you need to obtain a copy of your medical records a release is necessary. We require five-seven business days to process this request. The fee is $1.00 per page up to $100.00 with a $10.00 minimum. (Download form here) The Spine Institute maintains compliance with HIPAA regulations regarding confidentiality.

If you have downloaded the release form, you can fax it to 856-797-3637 when it has been completed.

Disability Forms

Disability forms require five-seven business days for the completion of the forms and there is a $15.00 fee for each form that is to be completed with the exception of New Jersey State Disability. If you have not had an office visit within the past 60 days, you will need to make an appointment to re-evaluate the status of your disability before we can complete any additional disability requests.
